POSITION SUMMARY:

We are looking for an experienced and compassionate Psychiatric Nurse Practitioner to provide high-quality mental health care across various settings, including Skilled Nursing Facilities (SNF), Long-Term Care (LTC), Assisted Living (AL), and Independent Living (IL). This role emphasizes work-life balance and offers a fulfilling opportunity to make a difference in the lives of adults and geriatrics in senior living communities.

As a Psychiatric Nurse Practitioner, you'll work closely with a multidisciplinary team to ensure continuity of care and tailored psychiatric support across the post-acute and long-term care continuum.

Key Responsibilities:

Psychiatric Care and Rounding : Conduct patient rounds and manage psychiatric care within SNF, LTC, AL, and IL settings.
Collaborative Care Coordination : Partner with attending physicians, consulting specialists, referral sources, and facility staff to ensure comprehensive, coordinated care.
Patient Evaluation and Management : Assess and manage the mental health needs of residents, perform physical exams, and gather complete medical histories.
Diagnostics and Therapeutics : Order and interpret lab and diagnostic tests, make preliminary diagnoses, and develop effective therapeutic care plans for mental and physical health.
Medication and Treatment Management : Prescribe and administer medications and treatments; provide injections as necessary.
Patient and Family Education : Educate patients and families on medications, care plans, and preventive mental health practices.
Acute Care Collaboration : Work alongside physicians and facility teams to address acute psychiatric and medical needs promptly and effectively.
Qualifications: Graduation from an accredited Nurse Practitioner program and ANCC certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
Active DEA license.
BLS and ACLS certification.
Current Nurse Practitioner license in the state of practice.
