Description: JOB TITLE: Outreach Case ManagerPROGRAM AREA:
Street Outreach JOB OBJECTIVE: To actively participate in the
crisis to housing continuum at Oasis. This position specifically
provides street canvasing, assessments, safety planning, case
management and coordinated entry services to homeless/street youth
in the Metro Nashville area. Salary Range: $39,000-42,000 Based on
Education & Experience Direct ServiceEssential Functions: - Provide
screening, assessments and safety planning to youth and young
adults who are experiencing homelessness or have runaway. -
Participate in street outreach canvasing and street-based case
management for RHSY (runaway/homeless/street dependent youth) in
the community. - Assist youth in creating plans to leave/avoid
homelessness and support them in goal obtainment - Provide
education and information about resources for RHSY to identified
community partners as needed. - Help to build and implement strong
internal collaborations that maximize Oasis resources for young
people. - Facilitate problem-solving sessions to overcome obstacles
and resolve conflicts involving youth - Assisting with volunteers
to provide essential services such as meals, supplies, programming
and outreach activities as needed. - Transport clients as needed
for appointments such as identification, health care, housing and
education. - Provide supervision to youth and young adults in
Drop-In Center during service center hours - Assist in setting up,
organizing and cleaning Drop-In Center - Assist in organizing
community donations and organizing clothing closet - Facilitate the
referral process for clients. - Assist youth in young adults
through the CE process - Attend weekly staffing/meetings and
regular supervision. - Document client interaction in a consistent
and ethical manner. - Participate with supervisor and appropriate
staff on specific case related problems. - Assist in preparing
clients for housing such as conducing housing assessment and
obtaining documentation necessary. - Assist youth formally and
informally as they master life skills, included but not limited to:
job readiness, communication skills, transportation, budgeting. -
Provide referral services and follow-through with appropriate
adjunct services, including: mental health services, medical,
housing, etc. as needed. - Learn and operate from a trauma
informed/positive youth development approach to foster a safe
healing environment to enable youth to overcome their life
challenges - Provide after-care services to youth after they have
moved to a safer living situation - Participate in after hours
on-call rotation as needed - Related tasks as
assigned.Non-essential Functions: NoneAdministrationEssential
Functions: - Document client interactions in a timely, consistent
and ethical manner. - Support Oasis staff in learning and accessing
resources in community available to connect youth to housing. -
Attend community meetings for networking and resource updates. -
Attend By Name/Care Coordination Meetings. - Attend other agency
networking/coordination meetings as needed. - Effectively capture
data and track outcomes in agency database and HMIS. - Maintain
consistent and timely records, including relevant reports to meet
funding and licensing requirements and document program spending. -
Prepare files and collect data on residents as assigned. - Conduct
follow-up contacts, as needed. - Participate in staff and program
development by attending trainings and giving feedback. - Attend
weekly supervision and weekly team meeting. Non-essential
Functions: - Serve on intra-agency and related community committees
when requested or representation is significant to the agency.

Minimal Requirements / Performance Standards: - Ability to
effectively communicate in oral and written form. - Ability to
monitor client interaction visually and auditorily. - Ability to
maneuver stairs and uneven terrain. - Ability to form strong,
caring and affirming relationships with all youth - specifically
including LGBTQ+ youth. - Ability to apply Motivational
Interviewing principals and operate from a Trauma Informed Care
approach. - Clear understanding of racial inequities and
willingness to operate from an anti-oppression model. - Knowledge
and ability to access community resources. - Knowledge and ability
to apply Positive Youth Development approaches and an understanding
of adolescent development. - Ability to deliver services through a
low barrier approach. - Valid driver's license and good driving
record. - One-year relevant experienceEducational Requirements:
Bachelor's degree in a human service field. Supervisor: Outreach
